What we collect
When you use a form on this site, we collect your first name, last name, email address, phone number, and ZIP code. We collect nothing else. There is no tracking pixel, no advertising cookie, and no third-party analytics script that identifies you.
Why we collect it
So that Terry Steaple, a licensed insurance agent, can contact you about health insurance options. That is the only reason.
What we will never do
- We do not sell your information.
- We do not share it with lead buyers, aggregators, or marketing networks.
- We do not use it to advertise to you anywhere else.
Where it goes
Your information is stored in GoHighLevel, the customer relationship platform we use to manage inquiries, and is accessible only to Terry and to people working directly on his behalf to respond to you.
How to reach us, or ask us to delete it
Call the number in the footer of this page, or email terry@mrterryhealthinsurance.com. Ask us to delete your information and we will.
Health information
This site never collects a Medicare or insurance member ID number, a health condition, or plan details — not through this form, not anywhere on this site. If Terry ever needs that kind of information from you, such as confirming you remain his client for renewal purposes, it is collected through DocuSign, a separate secure system built for that purpose, never through a web form or email.

If this policy changes, the updated version will be posted here.
